Case Manager conducts a Social Work Psycho-bio-social history Intake which examines a Strength-based Ecological, Systems Approach in relationship to Person in Environment
Case Manager and client develops a mutually agreed upon Service Plan with specific goals and objectives.
Case Manager gets clients permission to make referrals in accessing requested resources. Signature might be required.
If agency expects a written report, get client approval.
Case Manager schedules with client’s another appointment with Date? Time? Location? With whom?
Step 4: Follow-Up on Client’s
Service Plan
After Initial Interview, case manager writes up Their Initial Assessment: Psycho-bio-social history . Needs to be done in a timely fashion. In the report, the Case Manager includes any pertinent information received by the client or referring agency.

Awareness of individualized need, talking books versus scanning of text books due to student’s learning style.
Awareness stated problem with referral, client is under stress due to oral examinations, while client ranked what would help most is access to text books.

Step 7: Submit written report to agency’s team or supervisor
Aim is to assess, review, and evaluate pertinent information, Service Plan, and recommendations.
Case Manager shares impact and significance of this Case Study
If team or supervisor requests additional information or recommended referrals, then case manager must record case notes on this meeting requiring follow-up. Either with client or a resource agency for consultation, advise, or available resources…and methods to achieve these resources. This meeting helps to stage the next contact. Case manager determines if done by phone call or wait for the next session.
Step 8: Case Manager follows up on Initial Intake and Service Plan
Contacting Referring Agency with an update report or a face-to-face meeting with client and case manager. Log in date, time, location, contact person, main activity, and outcome results.
Accessing resources that addresses the most immediate need of the client in the initial interview. Done by calling or visiting the site . If needed, include the client in this process or linking client with the resources. Might need to practice this approach with client in your second interview.
